1. Athletic Physical - Physicals are good for 2 years

2. Parent Consent and Code of Conduct form signed by student and parents

3. Proof of Insurance Coverage

4. Athletic Fee $100 per sport

5. Impact Baseline Concussion Tested - Call Athletic office for testing schedule
