Athletic Registration

Klamath County School District has partnered with FinalForms, a secure online platform that allows parents and guardians to complete, sign, and manage athletic forms entirely online.

Flexible access: Review and update your student’s information at any time.
Annual compliance: Forms must be signed once per year and anytime updates are made.

Who needs to register?

All junior high and high school students planning to participate in athletics must be registered through FinalForms by a parent or guardian.

Get started:

• Visit Finalforms
• Create your parent/guardian account
• Add your student(s)
• Complete and sign all required forms

1. Register and complete all forms in Finalforms including uploading a copy of your completed physical form. 
2. Pay Athletic Fee of $100 per sport
3. Get Impact Baseline Concussion Tested - Call Athletic office for testing schedule

Note: Athletic fees and concussion tests are only required for high school athletes.
